The expiration date of National Health Insurance Certificate and Post Elderly Health Insurance Certificate you use now is July 31 (Sun.). So new insurance certificate valild from August 1 will be sent in Tokutei kiroku yubin (Special Recording System Mail; delivery to the mail box; record of the delivery with its date is given to the sender) in the middle of July.
* Please return your old insurance certificate at the window of the City Hall or cut it and dispose.
For the Members of National Health Insurance Certificate (Card) from 70 yrs to 75 yrs
"Hokenshou Ken Korei Jukyusha-shou (Health Insurance Card and Aged Recipient Proof)" shown your
co -payment ratio according to your income will be sent.
When you chang your name, householder's name or address etc. on your health insurance certificate
Take a procedure at the shimin-ka (Registers section) in the City Hall or at the Service Centers.
New health insurance certificate will be issued after the procedure.
