Necessary Report on National Health Insurance

When You Enroll in or Withdraw Health Insurance of Workplace
Take necessary procedure at the Hoken Nenkin-ka (Health Insurance and Pension Section) in the City Hall or at the nearest service center if you fall under any case in the following table.

Necessary things for the procedure

  • Required documents in the table below.
  • Identity verification document (Driver's license, passport, My Number Card, etc.)
  • Notification card (paper) or a (plastic) Individual Number Card (My Number Card) to show your Individual Number (My Number)
  • If an applicant isn't a member of the same household, "a letter of attorney" is necessary. The health insurance card will be sent to the address of the person in question.

Report immediately when you enroll or withdraw National Health Insurance Program

When you are late in reporting the enrollment of the national health insurance, the national health insurance tax will be laid, beginning in the month you became eligible for the national health insurance (such as the month you withdraw the health insurance of the workplace), so you have to pay the insurance tax for the past months.

If you do not report immediately, you are regarded to be continuing the national health insurance and you may have to pay both of the health insurance premiums (workplace and national).

Enrollment of the National Health Insurance

When you moved in from another municipality The certificate of moving out from the previous city (ward, town, village)
When you terminated the health insurance of the workplace The retirement certificate (unemployment slip), or the certificate of the loss of qualification for the health insurance
When you stopped being a dependent of the health insurance of the workplace The certificate of the loss of qualification for the health insurance
When a baby was born The health insurance certificate (card) of the householder and the document to certify the birth
When you stopped receiving Seikatsu-hogo (Public Assistance: welfare benefits) The notice of decision to terminate Public Assistance
When a foreigner enrolls in Residence Card, Passport

Withdrawal of the National Health Insurance

When? Required documents
When you moved out to another municipality The health insurance certificate(card)
When you enrolled in the health insurance of the workplace The health insurance certificate(card) of the National Health Insurance and of the workplace (or a document to certify the enrollment in the health insurance, in case the new health insurance card is not available)
When you became a non-working dependent of the workplace NOTE that the workplace will take care of the enrollment in the social health insurance, but will not terminate the National Health Insurance. You need to take care of it yourself.
When a subscriber to the National Health Insurance died The health insurance certificate(card) of the deceased and the document to certify the death
When you started receiving Seikatsu-hogo (Public Assistance) The health insurance certificate(card) and the notice of decision to start Public Assistance

When you separate or share the household
