Application for Expensive Medical Charges

Procedures Simplified
No Need for Application for the Second Time onward

In the national health insurance, expensive medical charges are granted in case the medical charges in a month exceed the limited amount, after application procedures. From November on, the application for simplifying the procedures will make it unnecessary to go through procedures from the second time on. The application forms will be sent to eligible families.

Necessary documents for application

• Kogaku Ryoyohi Shikyu Shinsei Tetsuzuki Kansoka Shinseisho Ken Doisho (Application form to simplify the procedures for the application to grant expensive medical charges and written consent)
• A document to show the bank account of the householder
• Health insurance card

NOTE

A separate application is necessary for the not-yet-applied expensive medical charges received in November or before. Simplification cannot be started for the families who have not paid the insurance tax fully.
