<1>Who will receive the envelope of the "disaster safety name list"?

Those who fall under (1) or (2) below, as of August 30

(1)Those who are registered in the Basic Resident Register, live in the registered address and all the members fall under any of the below:

• Yo-kaigo (requiring nurse care) level 3 or over
• Having Shintai Shogaisha Techo (physically disabled person's handbook) Grade 1 ~ 3 (except those whose disability are all in upper limbs)
• Having Ryoiku Techo (rehabilitation certificate) ○A, A or B
• Having Seishin Shogaisha Hoken Fukushi Techo (mental disability certificate) Grade 1 or 2

(2)Those who are certified as a recipient of Tokubetsu Shogaisha Teate (heavily handicapped person allowance)

NOTE: Contact Fukushi Seisaku-ka (Social Welfare Policy Office) if you fall under any of the below:

• You are eligible to receive the envelope but have not received one before December.
• You moved in Soka City on August 31 or later, and are eligible for this system.
• You received the envelope but have not yet returned the document.
• You are not sure if you returned the document or not.
