* Those of age 12 ~ 15 need the parent’s consent, and are to be accompanied by a parent.

- There is some availability for reservation for individual vaccination venues (medical institutions in Soka City) [Pfizer Inc.] and group vaccination venues [Moderna Inc.].
- Vaccination will be given until September 30, for the 1st, the 2nd and the 3rd time vaccination.
* The vaccination at the AKOS Hall will end on April 26 (Tue).

- For those who will be five years old, the vaccination voucher will be sent a few days after the birthday.
- The vaccination is not obligation to make effort. Consider the merits to be expected by vaccination and demerits by side reaction and others, based on the information on the website of Soka City and of the Ministry of Health, Labour and Welfare, and make a decision on the own will of the child and of the parents.
For Those Who Moved in Soka City
- The vaccination vouchers issued at the former address may not be used.
- Application is necessary to have the vaccination vouchers issued regardless of the times of vaccination.
- Visit the website of Soka City for procedures of application.
