Subsidy for High Medical/ Nursing Care Expenses

If you paid for both of medical treatment and nursing care service over your "Self-burden" during
August, 2020 through July, 2021, part of your expense is repaid.
Please check your "Self-burden amount" in the table below.

【Expenses not covered】

  • Exceed amount is less than 500 yen
  • Amount repaid by the subsidy program of Disabled Medical Insurance
  • Expenses not covered by health insurance; such as meal fees in case of hospitalization
  • Amount repaid by high cost medical treatment or nursing care service fee
  • You cannot add the expenses by different health insurance together even in the same household

A person who is enrolled in Kokumin Kenko Hoken (National Health Insurance)/ Kouki Koureisha Iryou Seido (Post Elderly and Disabled Medical Insurance)

Eligible person who are enrolled in these insurance programs will receive an application form from the end of March.
Please apply with necessary things listed on an application form.

A person who is enrolled in medical insurance except National Health Insurance/A person who changed his/her medical insurance

A person who is enrolled in medical insurance except National Health Insurance/Post Elderly and Disabled Medical Insurance or a person who changed his/her medical insurance during target period (including a person who transferred to Post Elderly and Disabled Medical Insurance when he/she reached 75 years old) need to apply to his/her medical insurance as of July 31, 2021 with necessary documents.
If you need a Jiko-futan-gaku Shoumei-sho (self-burden certificate) of Nursing Care Insurance, apply at Kaigo Hoken-ka (Nursing Care Insurance Section) with your medical insurance certificate, a certificate of Nursing Care Insurance, your bankbook and seal.
A person who transferred to Post Elderly and Disabled Medical Insurance from National Health Insurance doesn't need a Jiko-futan-gaku Shoumei-sho (self-burden certificate).
