List of Persons Requiring Assistance in Evacuation

Register to Prepare against Disaster

Soka City is making a list of names, based on the documents returned from citizens, so that people who will have difficulty in evacuating by themselves (people requiring assistance in evacuation) will have prompt assistance in evacuation in times of emergency.

The information registered on the list, such as names and emergency contacts, will be shared and used with related organizations who provide support in evacuation, including welfare commissioners and the council of commissioned child welfare volunteers, the police, the Self Defense Force, fire stations, etc.

Target People
Those Who Fall under the Condition (1) or (2) Below as of August 30

(1)Those who are registered in the basic resident register of Soka City, who live in the registered address and all the family members fall under one of the conditions below

  • Those requiring nurse care: grade 3 ~5
  • Those who have the grade 1 ~ 3 of Shintai Shogaisha Techo (physically disabled person's hand book), except the cases that the disabled parts are all upper limbs
  • Those who have the grade ○A, A or B of Ryoiku Techo (rehabilitation certificate)
  • Those who have the grade 1 or 2 of Seishin Shogaisha Hoken Fukushi Techo (Mental Disability Certificate)

(2)Those who are qualified to be recipients of Tokubetsu Shogaisha Teate (special allowances for people with disability)

NOTE: If you fall under one of the conditions below, contact Fukushi Seisaku-ka (Social Welfare Policy Office).

  • Those who have not received the document by December
  • Those who moved to Soka City on August 31 or later and fall under any of the conditions to receive the document
  • Those who received the document but have not yet sent the reply
  • Those who are not sure if they sent the reply

Normal Times

For those who gave consent to the sharing of personal information registered on the list with related parties to support evacuation, the information will be used for watching out for their safety in normal times by welfare commissioners, the council of commissioned child welfare volunteers, etc.

To Be Enclosed in the Green Envelope

For families who have difficulty in obtaining information by e-mail or from the website of Soka City, the emergency information related to disaster, which is broadcasted on the disaster prevention and administrative radio system, will be sent by fax or by phone.

NOTE: The line may be busy in times of disaster and it may take time for the information to reach those families.

By fax, postcard or phone, contact 〒340-8550 Kiki Kanri-ka (Crisis Control Section), telling your name, address phone number and the means you wish to receive information (by phone or by fax).

You can make an application even if you have not received the envelope.
