* 2 Soka City defined essential workers as workers in the medical field, in the educational field and nursery facilities as well as care service providers, but now the definition is changed to the same level as that of Saitama Prefecture. (See the website of Saitama Prefecture for details.)

Reservation procedure
Make the reservation for two times on the Internet or on the phone.
- Reservation will NOT be received on the phone or at the counter of the City Hall, of vaccination venues nor of medical institutions.
- The 2nd time vaccination will be three weeks later in the same time zone and on the same venue.

Vaccination Vouchers Sent
Vaccination vouchers for those of age 12 and over were sent individually by early August.
If you have not received one or have lost it, dial the Soka City Novel Coronavirus Vaccination Call Center.
* The vaccination vouchers were sent to those who became 12 by August 1. For those who will become 12 later, the vouchers will be sent after the birthday.
If you have received the vaccination voucher, you can go to a large-scale vaccination venue of the nation or of Saitama Prefecture.
- Target age groups are different from venue to venue. Make a confirmation.
- Make a reservation on the Internet in advance. The municipal code of Soka City is 112216.
