About the reservation of vaccination

Reservation procedure

Make the reservation for two times on the Internet or on the phone.

• Reservation will NOT be received on the phone or at the counter of the City Hall, of vaccination venues nor of medical institutions.
• The 2nd time vaccination will be three weeks later in the same time zone and on the same venue.

If you have received the vaccination voucher,
you can go to a large-scale vaccination venue of the nation or of Saitama Prefecture.

• Make a reservation on the Internet in advance. The municipal code of Soka City is 112216.
• Target age groups are different from venue to venue. Make a confirmation.

Make an application by postal mail.

Those who need the relaxation of biosecurity measures in going abroad from Japan.

Necessary documents

• Application form(to be downloaded from the website of Soka City)
• A copy of the passport (the page which indicates the matters for identification) In case of using the former surname, the alternative surname or the alternative given name, attach the documents to verify those names.
• Your ID (such as a copy of the driver's license or of a certified copy of resident register)
• A copy of one of the three documents - the certificate of vaccination (vaccination voucher), of the record of vaccination or the backup of medical interview sheet
• A return envelope (large enough to enclose an A4-sized paper) Write the return address and put the necessary postage stamps on it.
• In case a deputy goes through procedure on behalf of the applicant, documents below are necessary in addition.
• An authorization letter
• A copy of the ID of the deputy
