Attach the application slip, distributed at Soka-shi Kanko Kyokai Jimukyoku (Soka City Tourism Association Office) in Bunka Kanko-ka (Culture and Tourism Section) (also to be downloaded from the website of Soka City), on the back of the photograph and send it to the office above to be postmarked by September 30 (Thu).
