Genkyo Todoke (Current Status) for Jido Teate (Child Allowance) / Tokurei Kyufu (Special Allowance for the family placed an income restriction)
Submitting the Current Status Report ("Genkyo Todoke" in Japanese) as of June 1 is necessary to receive the Child Allowance (Jido Teate) / Tokurei Kyufu (Special Allowance for the family placed an income restriction).
This "Current Status Report" is for checking child/ren's custody and income.
The application form of Current Status Report was sent to the eligible person in the end of May.
After filling in and putting a seal on the form, mail the form and required documents with the enclosed self-addressed return envelope by the end of June.
If you will forget to submit the Current Status Report you may not receive the allowances in October for the allowances of June, July, August and September.
Required documents (except Current Statur Report Form)
(1)If you and your child(ren) are living separately in the resident regisitration.
Written Motion on child's custody and living equivalence (enclosed in an envelope)
This document is required when parents are living separately from thier children. If a child is living except Soka City a resident record (jyumin-hyo) of the child's household is required.
(2)If you and your child(ren) are not your biological child(spouse's child, grandson/daughter etc.)
Written Motion on child's custody and livelihood condition (enclosed in an envelope)
