Infants with intellectual disability older than 3 years as of April 1, 2021, who can walk by him/her-self. (Infants born from April 2, 2013 to April 1, 2016)
Necessary documents
Nyuen-soudan-kai Moushikomisho (application form), Shinshin-jokyohyo (mental and physical condition sheet), Shindan-sho (medical certificate)
* Application form is available at Aoba Gakuen (Aoba School) and Kosodate Shien Center (Parenting Support Center) or from the website of Soka City and Information Site of Bokkurun for supporting your parenting.

Mail an application form (including necessary medical certificate) to "Aoba Gakuen" (address: 〒340-0013 Matsue 2-3-60) by 20th day of every month * should be postmarked on or before 20th of every month.
