Vouchers for welfare taxis and automobile fuel will be issued to those who have mental or physical disability and are having difficulty moving around, so as to support independent life and social involvement.
The vouchers to be issued will be either one kind.
Basicaly this year, we will accept the application forms and send the vouchers by postal mail in order to prevent spreading of COVID-19 infection.
Eligible People
- Shintai-shogai-sha Techo (Physically disabled person's handbook) Grade 1 ~ 3 (except those of grade 3 with disability of only upper limbs function)
- Ryoiku Techo (rehabilitation certificate) A ~ B
- Seishin Shogaisha Hoken Fukushi Techo (health and welfare certificate) Grade 1 and 2.
NOTE
* In case the eligible person is hospitalized or staying at facilities such as a special nursing home for the aged, at the time of application, go through the procedure after leaving the hospital or the facility.
* If the above eligible people who are registered as a citizens of Soka City but lives in other cities are not eligible.

How to Apply
Mail the necessary documents
to the Shogai Fukushi-ka (Disabled Citizens Welfare Section) if he/she is of age 18 or older
or to the Kosodate Shien-ka (Parenting Section) if the applicant is under age 18.
Necessary Documents
- For taxi tickets: Application form, Techo (Certificate)
- For car fuel (gasoline): Application form, Techo (Certificate), Shaken-shou (automobile inspection certificate), Menkyo-shou (Driver's Liscense)
* Send photo copies of the documents except an application form
* Application forms are available from the HP of Soka City or ask to the section in charge.
Vouchers will be sent from April 1 (Thursday)
Application at the window of City Hall starts from April 1 (Thursday).
