Eligible infants
Infants with intellectual disability older than 3 years as of April 1, 2021, who can walk by him/her-self. (Infants born from April 2, 2013 to April 1, 2016)
Necessary documents
Nyuen-soudan-kai Moushikomisho (application form), Shinshin-jokyohyo (mental and physical condition sheet), Shindan-sho (medical certificate)
For apply
Mail an application form (including necessary medical certificate) to "Aoba Gakuen" (address: 〒340-0013 Matsue 2-3-60) by February 19 (Fri.). * should be postmarked on or before February 19.
Please indicate child's name, address, contact details, the date you want on an application form. Enclose a report of physical and social life ability, medical report.
The consultation date is decided on first-come-first-served basis.
* Application form is available at Aoba Gakuen (Aoba School) and Kosodate Shien Center (Parenting Support Center) or from the website of Soka City and Information Site of Bokkurun for supporting your parenting.
