Temporary Reception Desks to Be Set Up

Vouchers for welfare taxis and automobile fuel will be issued in advance to those who have mental or physical disability and are having difficulty moving around, so as to support independent life and social involvement.
The vouchers to be issued will be either one kind.

In case the applicant fails to make an application during this period, apply on April 8 (Wed) or later.
If the applicant is under age 18, go through the procedure at Kosodate Shien-ka (Parenting Section), and if he/she is of age 18 or older, at Shogai Fukushi-ka (Disabled Citizens Welfare Section).

NOTE: The number of vouchers to be issued in advance or in April will be the same.
NOTE: Please wear a mask to prevent the infection of the novel corona virus.

Eligible People

Shintai-shogai-sha Techo (Physically disabled person's handbook) Grade 1 ~ 3 (except those of grade 3 with disability of only upper limbs function), Ryoiku Techo (rehabilitation certificate) A ~ B as well as Seishin Shogaisha Hoken Fukushi Techo (health and welfare certificate) Grade 1 and 2.

NOTE: In case the eligible person is hospitalized or staying at facilities such as a special nursing home for the aged, at the time of application, go through the procedure after leaving the hospital or the facility.

What to bring

Vouchers for welfare taxi: Techo (the original handbook or certificate)
Vouchers for automobile fuel: Techo (the original handbook or certificate), the automobile inspection certificate and the driver's license

NOTE: If you wish to have someone write the form on behalf of you, bring also the seal.

NOTE: Vouchers will not be issued in the City Hall from March 23 (Mon) to 25 (Wed).
NOTE: Plan extra time for waiting because crowdedness is expected on all the days, especially in the morning.
