Soka City carries out "Tokutei Kenshin (Specific Health Checkup)" for citizen between the ages of 40 and 74 who are enrolled in National Health Insurance and another Checkup for the citizen who are enrolled in Post Elderly and Disabled Medical Insurance of 75 years and older.
Checkup in this fiscal year is available until December 18 (Fri.), 2020.
The persons as the table below are eligible for this checkup program.
As checkup coupons in blue envelopes have sent to the eligible persons in the end of May, please have a checkup soon.
If you lose the checkup coupon ask for re-issue.
