The expiration date of National Health Insurance Certificate and Post Elderly Health Insurance Certificate you use now is July 31 (Fri.).
So new insurance certificate valild from August 1 will be sent in the middle of July.
National Health Insurance Certificate (Card) (for the people under 75 yrs)
New insurance certificate valild from August 1 will be sent in Tokutei kiroku yubin (Special Recording System Mail; delivery to the mail box; record of the delivery with its date is given to the sender).
●If you want a mail in Kan-i kakitome yubin (Simple registered mail; hand a mail to the receiver directly; the record whether the recipient receive or not is given to the sender) apply at Hoken Nenkin-ka (Health Insurance and Pension Section) or service center nearest you with 160 yen stamp by June 26 (Fri.).
●If you have enrolled in other health program, you need to submit the cancellation of National Health Insurance.
Bring
(1)new health insurance certificate from your office
(2)National Health Insurance Certificate
(3)Individual Number (My Number) Card(plastic) or Notification Card (paper)
to Hoken Nenkin-ka (Health Insurance and Pension Section) or service center for the peocedure.
Post Elderly Health Insurance Certificate
New insurance certificate will be sent in Kan-i kakitome yubin (Simple registered mail; hand a mail to the receiver directly; the record whether the recipient receive or not is given to the sender).
Please return your old insurance certificate at the window of the City Hall or cut it and dispose.
