When You Enroll in or Withdraw Health Insurance of Workplace
Take necessary procedure at the Hoken Nenkin-ka (Health Insurance and Pension Section) in the City Hall or nearest service center if you are subjected in the following table.
Necessary things for the procedure
- Required documents in the table below.
- Identity verification document (Residence Card, driver's license, passport etc.)
- Notification card (paper) or a (plastic) Individual Number Card (My Number Card) to show your Individual Number (My Number)
- If an applicant isn't a member of the same household, "a letter of attorney" is necessary.
Report immediately when you enroll or withdraw National Health Insurancde Program
When you terminated the health insurance program of the workplace or stopped being a dependent (see the table below), you need to enroll in the National Health Insurance. And from the month national health insurance tax will be laid. If you will not report immediately you have to pay health tax backward the day you leave the company insurance program. Or you may pay both of the health insurance premiums (company and national) if you miss reporting withdraw of the National Health Insurance.

When you became a non-working dependent of the workplace | NOTE that the workplace will take care of the enrollment in the social health insurance, but will not terminate the National Health Insurance. You need to take care of it yourself. |
When a subscriber to the National Health Insurance died | The health insurance certificate(card) of the deceased and the document to certify the death |
When you started receiving Seikatsu-hogo (Public Assistance) | The health insurance certificate(card) and the notice of decision to start Public Assistance |
